Medical bills can be complicated. We want to help you understand your bills, our payment policy and your options. 


Payment Policy 


1. We will file insurance for our patients. However, all co-payments and/or deductible amounts are due to the time of service. Any disallowed amounts are due from the patient.


2. There will be a $32 fee assessed for any returned checks. This fee is assessed regardless of whether the check is re-desposited, as the bank has already charged our company a fee for the returned check, you will subsequently receive a bill for this amount.  


3. Any amount your insurance does not cover will be your responsibility as the patient to pay. 


4. No show visits will have a charge of $25. 


Your insurance policy is a contract between you and your insurance company. It is important that you understand what provider services are or are not covered before seeing your provider. We cannot guarantee payment of claims by your insurance company. Reduction or rejection of your claim by your insurance company does not relieve the financial obligation you have incurred.

What do I need to bring with me when I come to Mingus Medical Care, PLLC?

A photo ID such as a driver's license.

Your current insurance card (both primary and secondary insurance coverage).

Method of payment for any copayments, deductibles, or coinsurance. 


What if I do not have insurance?

The initial office visit is $95.00, follow up visits are $85.00

 

Is Mingus Medical Care "In Network"?

Before a visit, we encourage families to contact their insurance company directly for more information. "In network" means your insurance company has a contract with our provider with small out-of-pocket expenses. You may choose to see an "out of network" provider, but this usually means you may be responsible for a higher percentage of the bill.


We currently are not in network with Cigna or Health Net. We are working on these contracts. 

As a courtesy we will bill all types of insurance for you.


Why do I still owe a balance if my insurance company has paid?

Based on your insurance plan, you may be responsible for a co-pay, deductible, or co-insurance percentage. 


I have a question that's not on this list. How do I get answers?
